Care Coordinator - Part Time (20 hours per week)

Location

Remote, available 8:00/9:00 am EST to 4:00/5:00 pm EST (with some flexibility)

Summary

Mellie is a ֵcomprehensive solution for family caregivers that assists them with navigating and managing elder care. Through the use of an innovative platform and experienced elder care professionals, Mellie provides family caregivers with care planning, resources, information, and tools for collaboration and communication so that they can provide the best care to their loved ones and to themselves.


As a Care Coordinator, you will work directly with Mellie's families to support them in caring for their older loved one. This includes assessing their needs, developing care plans, identifying and organizing resources, and providing concrete assistance with coordinating care and completing tasks. You will report to the Director of Care and work closely with all members of the Mellie team.

Responsibilities

  • Develop client-centered care plans based on the intake assessment and update care plans as needs change.

  • Identify areas of need and relevant resources based on assessments and ongoing client communications.

  • Assist in the clients’ initiation of services including onboarding and engagement.

  • Provide practical support related to the coordination of care, navigating resources and advocacy.

  • Establish and maintain a care management relationship with the family caregiver(s) to provide support and drive engagement.

  • Assist clients in utilizing the Mellie platform and provide feedback to the Mellie development team.

  • Cultivate relationships with referral partners and assist with the vetting and maintaining of resources.

  • Maintain compliance with Mellie documentation practices and standards and participate in quality improvement processes and Mellie team meetings.

  • All other related responsibilities as assigned.

Qualifications

  • Bachelor's degree in social work, healthcare, or other related human services field. Masters degree in social work or gerontology preferred.

  • Minimum of 5 years experience in working with older adults and providing care management services.

  • Knowledge of resources and services for older adults and their family caregivers and experience navigating such resources.

  • Experience with and understanding of the medical and psychosocial problems of older adults and family caregivers.

  • Strong communication skills, both written and verbal, and ability to communicate effectively with family caregivers.

  • Ability to work in a self-directed manner while coordinating with team members.

  • Strong sense of empathy, compassion, and professionalism, as well as customer service.

  • Computer literate and comfortable learning and using new technology.

  • Bilingual Spanish/English preferred but not required.

Salary

$30 - $35 per hour, commensurate with experience
